Dr austin ~ Notice of Retirement

Dear Patients,

I wish to inform you that after 38 and a half years of practicing Family Medicine, I have decided to retire. I will be retiring from the practice of medicine on November 27, 2025.

I feel very fortunate to announce that Dr Olujide Soremi and Dr Emeka Aguiyi will be taking over my practice. Drs. Soremi and Aguiyi are members of the Royal City Family Health Organization, under the umbrella of the Guelph Family Health Team.

Dr Soremi received training at the Royal College of General Practitioners in the United Kingdom, completing his residency training in 2014. He is currently a member of the Canadian College of Family Physicians and an Assistant Clinical Professor of Family Medicine at McMaster University.

Dr Aguiyi also trained at the Royal College of General Practitioners in the United Kingdom, completing his residency in 2019. He is currently a member of the Canadian College of Family Physicians and, an Assistant Clinical Professor of Family Medicine at the Michael G. DeGroote School of Medicine at McMaster University.

Drs Soremi and Aguiyi’s office is located at: 83 Dawson Road, Suite 101, Guelph Ontario, N1H1B1.

Your medical records will automatically be transferred to their office upon my retirement. Should you choose to make alternate arrangements for your medical care prior to my retirement, please provide written authorization to my office for your records to be transferred to your new physician using the form below, no later than November 20, 2025. After November 27, 2025 any requests for transfer of records should be directed to the office of Drs Soremi and Aguiyi at 83 Dawson Road.

I recognize that changing not only family doctors, but health care teams, can be stressful. However, I feel gratified to be able to transfer my practice to such caring and competent doctors.
