Dear Patients,

I wish to provide you with some updated information about my retirement on November 27, 2025.

The previous announcement stated that Drs Soremi and Aguiyi would be taking over my practice. Due to unforeseen circumstances, Dr Aguiyi alone will assume my roster.

Dr Aguiyi trained at the Royal College of General Practitioners in the United Kingdom, completing his residency in 2019. He is currently a member of the Canadian College of Family Physicians and, an Assistant ClinicalProfessor of Family Medicine at the Michael G. DeGroote School of Medicine atMcMaster University.

Dr Aguiyi’s office is located at: 83 Dawson Road, Suite 101,Guelph Ontario, N1H1B1.

Your medical records will automatically be transferred to their office upon my retirement. You do not need to complete a release of information if you intend to transfer to Dawson Road. Should you choose to make alternate arrangements for your medical care prior to my retirement, please provide written authorization to my office for your records to be transferred to your new physician using the form attached, no later than November 20, 2025. After November 27, 2025 any requests for transfer of records should be directed to the office of Dr Aguiyi at 83 Dawson Road.

Please note:
All of the doctors in my current office have full practices, and are unable to accept patients at this time.
